Family Health Clinics in the Leeds Grenville Area
Kemptville District Hospital (KDH)
Located in North Grenville, Kemptville District Hospital (KDH) is a fully-accredited health care facility committed to building healthy communities. It offers a full range of services including an emergency department open 24 hours a day. A National Research Corporation Canada Report entitled Patient Ratings of Overall Care and Likelihood to Recommend Hospitals (2014) placed KDH in the top 10 per cent in Ontario for inpatient care experiences. Patients rated the hospital in categories of patient satisfaction and for the overall care they received and their likelihood of recommending the hospital to family and friends. KDH opened a $24-million expansion of its facility in 2010. The expansion included state-of-the-art operating rooms, the Tallman Family Emergency Department and new diagnostic imaging equipment. Kemptville District Hospital was rated in the top 10 per cent for its Emergency Department in 2013.
In 2012 and through a unique partnership with Ottawa Hospital, KDH launched its Total Joint Replacement orthopaedic surgery program. It has since been recognized by the Ontario Hospital Association. KDH also earned the highest level of satisfaction in Ontario for its emergency room, day surgery, inpatient surgery and medical care. Brockville General Hospital (BGH)
Brockville General Hospital is a four-site community hospital with over 129 years of service to the United Counties of Leeds and Grenville, serving a catchment area of 66,000 residents and a referral area of up to 125,000 people.
The BGH Charles Street Site offers full acute care services, including emergency and intensive care, medical, surgical and obstetrical care. Various ambulatory clinics are also available, along with day surgery, ECG, stress testing, radiology, ultrasound and an Ontario Breast Screening Clinic. The BGH Garden Street Site specializes in complex/palliative care and rehabilitation. In addition to these, BGH sponsors an Assault Response and Care Centre.
BGH assumed governance of mental health acute care services for Leeds, Grenville and South Lanark in 2012. These services include the Assertive Community Treatment (ACT) Team of Leeds, Grenville and South Lanark, and the Mental Health Crisis Team of Leeds & Grenville, which provides support to the BGH Emergency Department, inpatient and outpatient services. Community and Primary Health Care (CPHC)
Community and Primary Health Care provides accessible and primary health care and community support services in Leeds Grenville and Lanark counties. The centre also offers adult day services, a diabetes education centre and Lifeline, a personal response system which allows the subscriber to call for help 24 hours a day. Lifeline is also available in Kingston through CPHC. Other services include Diners Club, Friendly Visiting, Meals on Wheels, Value for Life, transportation to medical and social appointments and various clinics, to name a few. CPHC also operates a traveling mobile unit which visits rural areas (see the photo below).
The CPHC's Family Health Team is comprised of family physicians, nurse practitioners, registered nurses, social workers, dieticians, psychiatrists, a physician assistant and support staff. Upper Canada Family Health Team
Upper Canada Family Health Team is comprised of a group of health care professionals working collaboratively to provide comprehensive and accessible health care. The goals of the Upper Canada Family Health Team include preventing illness and promoting the health of patients. The aim to reduce the number of patients in Brockville who do not have a health care provider, reduce ER visits for minor illness, and reduce hospital admissions for chronic illnesses.
