Special Need/Social Need Recommendation Form |
There are situations where licensed child care may be identified to meet a child’s need for optimal growth and development, where the parent(s) is not participating in the workforce or pursuing education/training. In this case, the family may still be able to access child care fee subsidy to assist with the cost of licensed child care. To be considered for child care fee subsidy eligibility, where the parent(s) are not working or going to school, the applicant must have a health professional/social agency complete a Special Needs/Social Need Recommendation form. The social agency/health professional recommending child care would complete the form if it is believed that the child’s participation in a licensed child care program would benefit the child, alleviate the crisis and/or significantly reduce stress on the family. |
Continued Eligibility |
Parents are required to provide updated Notice of Assessment information as well as proof of employment/education annually to ensure continued eligibility for child care fee subsidy. Should your annual income and/or your work/education situation change, your eligibility for child care fee subsidy may also change. Once approved for child care fee subsidy, applicants are responsible for notifying their Case Manager of changes in contact information, address, income, and/or hours of work or school. Failure to do so may affect your eligibility for fee subsidy and result in an overpayment. |
