Integrated Program Delivery Department Customer Service Survey

The Integrated Program Delivery Department is conducting a Customer Service Survey to find out what you think about our services. If you currently receive Ontario Works or Child Care Fee Subsidy assistance this survey is for you.

Your input will be used to plan and improve the services we provide. The survey will take about 5-10 minutes to complete.

 

To thank you for your time in completing the survey, you have the chance to be entered into a draw for an iPad Mini 2. To keep your responses confidential, the draw entry will be separated from your survey responses. Please complete only one survey for your household. Submission of extra surveys will not increase your chances of winning the iPad Mini 2 tablet.

Completing the survey is voluntary and your information will be kept confidential. Your name will not be recorded with the survey, and only a summary of all answers will be used.

If you do not want to complete the survey online, a hard copy of the survey is available at each Community and Social Services reception.

 

Surveys will be accepted from June 1 to July 31, 2017.
